I think you make some very good points Chris. I have stopped limping in EP with 22-55. I think it's -EV. Since these are the lowest pairs, set over set happens most often with these pairs. In fact last night, I just flopped a set of deuces to a set of fours and lost a full stack. If you go broke playing 66-JJ and lose set over set, then that's another story. In fact my 44 is only showing positive BB's/100 because I hit quad 4's vs. a guy's 5's full of fours. We both flopped a set on the flop and I turned quads. If I didn't hit my one outer, I'd be down on 4's as well. I think my numbers show that I'm positive with 66 and up. Of course, my AA is in the red because people are constantly hitting sets on ragged boards and I haven't been able to lay down AA on the flop. When the flop comes down 10-4-2 and 10-5-7 and 7-4-2 (all flops I've lost to sets with recently), you will be able to stack somebody with AA. I don't know how many people here can fold their overpairs, but I'm just getting crushed with AA on these boards. It doesn't matter if they lead into me, min raise me, or make a big raise. I'm losing every which way.
Medium pairs are cashcows in certain situations, but it definitely depends on position/type of game and other factors. Limping with all pairs from anywhere in all games and calling big raises isn't smart and a lot of people do play that way. But, many players on this site do realize these factors and don't advocate that type of play.
